mandates under the PPACA (Patient Protection & Affordable

first $1,000 of Emergency Rooms services or supplies incurred each calendar
year after a $150 access fee is paid (access fee waived in
Illinois). Any remaining costs subject to deductible and

benefit for X-Ray & Laboratory tests performed in a
Physicians office. With the "Premier Plus" plan there will
be a $25 co pay required per test; up to $200 per test then
